Windsor Dental, Vision Benefits Available to You

Helping you receive all of your healthcare benefits is important to us. That is why we want you to know about the dental and vision benefits available to you.

DENTAL BENEFIT

New to many of the Windsor Medicare Extra plans this year is the Dental Services benefit. Every year, plan members may receive regular cleanings, checkups, and any other dental services through the dental provider of your choice.

You may use your benefit for any type of oral care: dentures, teeth whitening, extractions, etc. You are not limited to just cleanings and checkups. The choice is yours up to the limits of your coverage. There are no pre-authorization requirements or benefit limitations. You may have your dentist process your claim for you by sending a copy of your invoice to our Claims and Submission Processing department at:

You will be reimbursed by submitting a service invoice and receipt to the claim address listed in the first column. Your plan reimbursement will not exceed $250 annually.

VISION BENEFIT

The vision benefit included with your plan is administered through our provider network with EyeMed, the only leading vision care plan with a choice of both independent providers and the nation’s leading retailers. Your annual benefit may be used for routine eyewear, which includes frames, lenses, and even contacts.

You will also receive an additional 20 percent discount off of the remaining balance—an added advantage of using network providers. (Services received outside Windsor Medicare Extra’s provider network will be your responsibility.) Limits apply. Not valid for any other use.

To locate an EyeMed provider in your area, you may do one of the following:

  • Visit www.WindsorExtra.com to view the latest provider directory or search for a provider in your area.
  • Call our Member Services department at 1-800-316-CARE (2273). We are available seven days a week from 7 a.m. to 8 p.m. CST to assist you. TTY users should call 1-800-848-0298.
  • Look in your printed provider directory that was mailed to you or supplied by your enrollment counselor when you enrolled.
