Windsor Medicare Extra is a product of Windsor Health Plan, Inc., a Medicare Advantage organization with a Medicare contract. Windsor Health Plan, Inc., renews its contract with the Centers for Medicare and Medicaid Services (CMS) annually. Benefits beyond the current contract year are not guaranteed. Even if a managed care plan leaves the program, you will not lose Medicare coverage.

Generally if you join a Medicare Advantage or Medicare Prescription Drug Plan, you can only change plans under certain circumstances. For more information regarding your election period options, please contact Customer Service at 1-800-316-2273 (TTY: 1-866-460-7617) 7 a.m. – 8 p.m. Central Time, seven days a week.

All routine care must be provided by plan providers, except in emergency and urgent situations, and for out-of-town renal dialysis. If you obtain routine care from out-of-plan providers, neither Medicare nor Windsor Medicare Extra will be responsible for the costs.

The Windsor Medicare Extra prescription drug benefit is available only to members enrolled in a Windsor Medicare Extra Medicare Advantage-Prescription Drug (MAPD) plan. If a beneficiary is already enrolled in an MAPD plan, the enrollee must receive his/her Medicare prescription drug benefit through that plan. Generally, pharmacy benefits are only available at the Windsor Medicare Extra contracted network pharmacies. In emergency or urgent circumstances, benefits may be obtained out-of-network.

Windsor Medicare Extra plan premiums vary by service area and/or plan. You must continue to pay your Medicare Part B premium if not otherwise paid for under Medicaid or by another third party. If you have qualified for additional assistance for your Medicare Prescription Drug Plan costs, the amount of your premium and cost at the pharmacy will be less. Once you have enrolled in Windsor Medicare Extra, Medicare will tell us how much assistance you are receiving, and we will send you information on the amount you will pay.
